Jessica holds a Bachelor of Health Science (Clinical Nutrition) from Torrens University and is registered with the Australian Natural Therapies Association (member no. 28236).

The information you have provided is to be used to advise dietary and lifestyle changes, as well as prescribe nutritional supplements. By signing this form, you state that the information provided is true and a current representation of your health status. Any changes to you state of health or medical diagnosis must be communicated as soon as possible. Whilst I will try, to the best of my ability, to help resolve your health issues, there is no guarantee regarding the outcome of your treatment.

CONFIDENTIALITY AGREEMENT

All records are kept private and confidential, and personal information will not be shared outside of your clinic session unless otherwise agreed to. Sometimes it will be necessary to share your personal health information to referring practitioners or your personal GP.

CONSULTATION POLICY

You give Well Balanced Health and Nutrition permission to conduct and treat you through online or in-person consultations. You understand that there are potential risks to online consulting, such as technical issues and interruptions. Well Balanced Health & Nutrition will not conduct a online consult with a person who is driving or moving around. You must have a quiet place to sit, a working camera and audio, and be able to stabilise your device. A consult may be discontinued and/or moved if a stable connection cannot be established.

CANCELLATION POLICY

You understand that the 50% deposit required for booking is non-refundable if you cancel within 48 hours of your appointment.

PAYMENT POLICY

An invoice for the remainder of your consult fee will be sent via email with 24 hours after your appointment. The remaining 50% of your consultation fee must be paid within 48 hours of receiving your invoice, or a $5 late fee will incur for each overdue day after your payment is due.

EMAIL POLICY

Any contact made with the practitioner can be done via email. Emails will be answered within 24-48 hours of being received. Please understand that Well Balanced Health & Nutrition can not continue consulting via email after your appointment, if you have a number further questions or concerns please make another appointment time. Email is only for urgent questions regarding your prescription, treatment plan, Vital.ly, new appointments, or payment.
